MaxFund Volunteer Hour Logging form

When possible, it is best to log your volunteer hours while you are at the shelter. We do understand that when volunteering at mobile adoptions and events outside the shelter it can make it difficult or time-consuming to log the hours, so we are offering this as an alternative. Submitting hours with this form will send the volunteer coordinator the hours and they will get input to the database in a semi-timely fashion.

A few points if you would like to log your hours using the online utility.

If you have multiple dates/multiple events to log, please submit the form for each event.
Before you can submit hours via the web, you must have entered yourself into the computer at the shelter. If this is not possible, please let Matt know.
Please make sure the name you enter matches the name in the system at the shelter exactly (Matthew Adams is NOT the same as Matt Adams which is not the same as Matt J Adams).
